::- Information to be filled as per documentary proof i.e. Birth Certificate & Aadhar Card.
::- Aadhar Card Number of your child, Father & Mother & Blood Group of your child.
::- Soft copy of Photograph of Child, Father & Mother (Should be less than 200KB) in JPEG Format)
::- Soft copy of proof of residence (Aadhar card/Ration Card/Electricity/MTNL bill/VoterID/Passport) (Should be less than 2 MB in JPEG/PDF Format)
::- Soft copy of Child Birth Certificate (Should be less than 2 MB in JPEG/PDF Format)
::- Soft copy of Proof of Sibling, if applicable - latest fee receipt. (Should be less than 2 MB in JPEG/PDF Format)
